A strange door not spawning
I have beaten mother. I have the achievement for unlocking the strange door and the one for unlocking jacob. But every time I get to depths 2 (not gehenna or mausoleum) the door is not there. I have done several runs and it hasnt spawned a single time.

Maybe some of your data reset from the online beta(if you have been playing it)

Go to your Stats screen from the main menu and press alt+F2 to re sync your unlocks to your achievements . If you have a graphics card overlay (Like NVIDIA Geforce) you may have disable that because it will try to take a screen shot instead of the game registering it. If it works you will see your number of secrets change.

I have had this problem several times and this has fixed it.
