My favourite way is to kill Isaac with ???

Keep an eye out for blue rocks and items that will prolong your health like rev, relic,miter,dark bum and hirophant cards. Blow up skulls in necro and depths.

If you're having trouble, beat Isaac five times with other characters to unlock the Polaroid. If ??? holds it, he'll get a shield for a few seconds every time he gets hit, which makes the Womb and the Cathedral a lot easier.

Of course, if you've already unlocked the Polaroid, then ignore me.

Ill try that once i die on here. I have that item that gives you a shield for the first hit on the room so that makes it a little easier.

Generally, ??? runs are dependent upon getting health. Finding an early Rosary or some early bombs can really help. Don't rush, and do your best not to get hit. Your first time to Cathedral with ??? is probably going to be a slow run, accept it and get ready to play slots for soul heart payouts.

generally, the more you play the game, the better you will be at getting father into the game. so if you're havinf trouble, just get some unlocks with other characters and when you feel like you've imroved enough, try again. Of couse, the other option is to keep trying and hope to get a really good synergy or item and auto-win, but that not all too likely.

Thanks for all the great advice! I was so close one time... I had the cross that blocks damage and the trinity shield but that was the run I unlocked it lives and i thought I had done it before.

Just keep grinding at it mega. I finally unlocked the d6 after dying to Isaac twice as ???, one time at less the 5%. One of the best bits of advice I can offer is to think over and possibly search item synergy before grabbing everything you see (Check out platinumgod.co.uk/rebirth for the best item cheat sheet if you havent already). I had ipecac on my winning run and couldn't tell you how many items dropped that would have thrown the run.

If you ever get the chance on a normal run and find the ankh you can kill yourself just before the boss fight and respawn as ??? and try to fight isaac with the items you got from the character you were before
