platinumgod.co.uk/rebirth has all of the information you need. Fate is unlocked by beating the boss Blue baby with the character Blue baby. In-game they are called ???, they are more commonly known as Blue baby in the community.

Fun fact: In the last game ???'s 'death note' said Blue baby, but in this game it actually says ???

No. In the last game, it had a 50% chans to able to show up at all during a run. It can now be found in every run as long as you have it unlocked. I think that's how it works.
