The Binding of Isaac: Rebirth

Only got soul hearts for a run
I was playing as Eden and had a normal run until I used a D6 room. After this i only got soul/black hearts, unless it was from the book of sin. Is there an item that does this, or what?
Also, do you need to die in order to activate Judas' shadow, because I picked it up and got Eden unlocks for beating Satan + Lamb

There are various items that increase the chance of reds spawning as soul or black hearts, most notably Mitre but also a bunch of other faith/evil items.

And yes you must die for Judas' Shadow, you respawn as the super powerful shadow Judas who has a 2x damage multiplier compared to the 1.0-1.35 of normal characters.

Judas' Shadow activates on death and turns you into Shadow Judas which is basically a much stronger version of Judas (even higher base damage (2x the isaac base damage of 3.5 rather than 1.35x of normal judas), more health, etc). Of course this would also affect which items are unlocked, if any, after it activates.

If the dice room re-rolled you (either 1 or 6 shown iirc) then it's possible that you were given a bunch of items such that it appears as though only soul hearts can spawn. One of the bigger ones is mitre, which alone gives a 50% chance of a heart being a soul heart. Another big one is the daemon tail trinket, which decreases heart spawns considerably while increasing the chance that any heart spawns will be black hearts.

Combined with the mitre the daemon tail trinket could quite easily make red hearts extremely rare and soul/black hearts uncommon. With the relic (and/or gimpy, if you get hit a lot) too it would make soul hearts common and black hearts uncommon while red hearts would be rare. Soul hearts are also the only type of heart that can be gained from blue/purple fires and tinted/marked rocks. Beyond that it'd be mostly down to luck I guess. The book of sin however should also be affected by the mitre/daemon's tail when it spawns a heart consumable in theory - though it could just be luck of the draw that when you used the book it just happened to land on the tiny chance of a red heart.
