how do you get platinum god or even real platinum god ?

All items unlocked = plat god (I think)

All items unlocked and collected = real plat god
You also need to be all the "main" bosses on hard I believe.

Platinum god is earned by unlocking every item EXCEPT the lost and all items that require you to play as the lost.

Real platinum is gained by unlocking and collecting every item at least once as well as every other achievement.

I think thats a bit of a glich, you do need to pick up all the items. For some reason, when you unlock bandage girl, it thinks that you have picked up the super bandage, even though you havent.

First you need to make sure to beat mom's heart on hard mode with all the normal characters, then unlock the lost and beat all floors on hard mode including bossrush. Next collect all items in the game and unlock all secrets. That is how you get The Real Plat God.

Ok :-) And i also get more items when i kill a boss multiple with one character right ? i did that with simon (killed mom's heart a second or third time with him) and got an item. how often do i need to do this with each character ?

The rewards for beating mom several times are of a general nature and it caps at 10 with, I think, Transcendence. Then there's a unique item for defeating several bosses with every character. Basically it's necessary to fill up the progression sheet oyu see in the character selection screen with all the marks. There are 7 marks in total.

10/10 guide learned everything I wanted to know and unlocked The 420

[Solo Death]

Platinum god you have to pick up every item in the game once except the ones related to the Lost, as well as get every achievement not related to the lost, including the ones for not taking damage on basement/caves/depths/womb/chest. Real Platinum God is all of those prerequisites, plus everything for the lost.

You'll know you've collected the items when it pops up saying you've achieved Platinum God/Real Platinum God in-game and an achievement will pop up from Steam, too.
