A thing I'd like to see in game
Zelda-like puzzles. You could find some secret rooms sometimes, like the ladders you can randomly find, where there would be a puzzle and where it requires you to think a little bit to get an item.

Isaac could have to play with some electric wires (Connect the lines)to unlock a special door. He could be wraped into the TV his mother was watching...
Some block pushing puzzles or what about a shooting range. I don't know, any kind of mini-game that you could thing about and that would fit into the Isaac's universe, mostly the cellar with the wood walls and the womb with it's organic setting.

The Boss Rush, MegaSatan and the secret character are great but I think it should be used in different and simpler ways without removing a possible challenge. And without removing these, obviously. Things like the dice rooms and the planked rooms are all great and should be used in more interesting ways.

Could be interesting, but the game isn't much about puzzles 'n stuff and I think it would break the flow of the game.

Except Zelda puzzles aren't challenges, they're time-wasters, and that's all these would be as well. 30 seconds after these are added, they'll be an image showing exactly how to beat them, making the puzzle itself pointless.

unless the puzzle was randomly generated each time, and depending on the floor you're on, the difficulty increases, as does the rarity of the item reward, which would be nice.

This. And I was thinking about something more secret. Not something that would break the flow, obviously.

I guess this isn't false but I'm still pretty sure you could implement something that feels like some kind of puzzle but isn't such a time waster.

I'm not saying "Add block pushing puzzles to the game"
I'm saying "Think about some kind of puzzle that could be great, that could fit in the universe and that isn't breaking the flow." Which resume pretty much my OP.
