I may be able to finally win the first part of the game (When you start).. I dunno, with 2 hearts and 1 blue heart xD (I got an item increasing damage in demon room) and THEN I got an extra life it looks like? Beside my hearts is a x1... what is this? That's the extra life right? How do I use it/or how do I use it when I die?

x1 next to your hearts is a extra life wich activates when you die

There are different extra life items but in general they resurrect you outside the current room when you die, sometimes as a different character. They are honestly not very good since you lose all soul hearts and might get kicked down to a lower amount of reds depending on the item.

If you die you respawn with 1 heart

No, it depends on the item. Ankh resses you as ??? with three soul hearts etc.
