Polycephalus is forced damage
You know that one I'm talking about, unfortunately I do not know the name of it but it burrows underneath the ground, is basically 4 heads with empty eyes, with blood around those eyes patched together and spews blood at you when it comes back up. It will appear in the room with very little space for you unless you have flight in which you can cross the blood river to lots of rocks. It is very hard to see where it is coming up next, I don't even know if you can tell. It is forced damage? It is not mega maw, scolex or pin.

Thats the one. How are you supposed to tell where he will come up next? I don't think there is a shadow, and he often telefrags me.

Try to get his speed down pat. Then try to "lure" him. Don't stop moving if you don't lure him immediately, but circle around where you think he is.
I don't think I've EVER been hit by him unburrowing, or hit by him specifically at all. But I sure as hell will tell you, lumps, that's a different story.

Agree, try to run in circles around the place where you think he is, he usually pops up. Still, sometimes he just stays down endlessly until you stop, and then spawn right under you, that's true. But it doesn't happen too often.

There is indeed a room on the Womb where he shows up, and there's no way to circle around him without flight. You have to run over him over and over again, and he usually pops up right below you.

If you're talking about the room that's shaped like an upside-down "u" with a hole being the letter, and rocks covering the lower half, once you learn his patterns, you can utilize the left and right sides of the room to avoid damage and lure him properly.

Honestly I've found the best way at this point is to move close (but not quite close enough to be hit) when he's still burrowing away (before the graphic disappears fully) and he'll pop out almost immediately. After the graphic has fully disappeared you're essentially blind to where he'll pop up, and it can easily end up underneath you if you're unlucky.

If you are a flying character then he sometimes won't come up until you move back into the area he can move in, as you'd like lose his position it's possible he will pop-up on you as you move back into his area. He should have a max time he is allowed to spend undercover.

Poly is a pretty easy kill for me. Like an ugly snake he has to be charmed so that he pops up where you want him.

My strat:
Visualise him making a b-line towards you whilst hes underground.
Travel fairly fast clockwise or anti-clockwise around the edges of the room (he won't pop whilst your doing this). When your ready for him to popup: stop and carefully take couple of steps towards the center. (this causes him to popup up infront of you) blast and dodge. - Repeat.
