Pyromaniac, it basically makes bombs into hearts. Not to mention other sources of explosions. Kinda useless on characters that can't have red hearts though... Or the stopwatch, which is hands down an item that can make you win a run regardless of anything other than the worst possible luck with teleporting enemies. I think I may even like the stopwatch more than pyromaniac, but it's just broken...

It slows everything down to 50% speed. This means enemy tears also move half the distance and most end game bosses can't easily reach you.

Stop watch, probably. I haven't got to use it because my donation machine isn't high enough, but it seems like the best.

Scythe. Its the only item I refuse to not pick up. Even if I have to trade all my hearts to the devil. Its the best item in the game imo.

Epic Fetus is still a firm favourite, even though I've only used it in the Glass Cannon challenge: they really ramped up the scarcity, didn't they? I wonder how well it synergizes.

I really like Infestation 2. You can spawn so many attack spiders, and, if you have high damage, bosses are super easy to kill.

I did the boss rush room with stopwatch slowdown on one character, what a pushover. Although it does have a downside once in a while, I'd argue it is the best item - moreso because you can just buy it every other run.

no? Dr. Fetus does barely any damage (without being buffed by other items), Epic Fetus is still monsterously powerful.
