I have had normal money beggars take that long before as well - you get a bunch of other consumables, chests, etc before the final item when they do that though.
Never had it occur with devil beggars, though I have used a lot of hearts on them and not gotten an item before (using shield effects to protect health and still losing most my health to it. I'd assume that there's an RNG factor for the amount of times for all 3, and sometimes it's a bit on the high side - it's not like you have to get the final item from them though, key beggars in particular often give more useful stuff from chests than the final item as they normally just give me the chest lockpicking trinket when they leave.
