A have a feeling it's basically down to the 1st item position not being easily movable when spawning a second one or something (and then it looking a bit odd when one is central and one is off-centre to the left or the right), but placing it exactly where a south door can be is a rather silly too, and is worse from a gameplay perspective.
I've seen screenshots of items spawning at the west door space as well (though I've never seen this happen myself - I assume that's down to an enemy death position or something, in which case it's fair enough and easily avoidable), which also seems rather silly - the last place you want a random item to spawn is a spot that you're forced to walk through to leave the room if you want to explore the rest of the floor and lack a teleport item. Not every item is a purely positive one and some normally good items also have actively harmful synergies with other items, so it's not like there's a fair few scenarios where you may want to avoid certain items, even more so when you consider re-rolling with D6/dice rooms.
