Essentially the game ends with Isaac in a chest consumed with the idea that he is full of sin and is becoming the devil. Isaac believes if he gets into the chest he will die and thus save the world from his sin. Note that the "real world" events are also twisted by isaac's imagination. He imagined that god defeated his mother but in reality that event never happened, to this degree the apperance of ??? and isaac's reflection of a devil and becoming a devil are likely his imagination.
