Edit: And you don't have to do anything special to unlock the item, it is in the defualt item pool, so if you get a good seed you can find it as early as your first run... obviously you have to get lucky finding it since doing a seeded run to guarenty the item showing up prevents Secrets from unlocking, and so even if I told you a seed that has Holy Mantel in the first item room so you could easily win with ??? with it, you wouldn't unlock the D6... but if you want the seed so you can practice with it,

also try to unlock the polirod before trying any ??? runs it helps greatly after the mom fight

Stores now seem to reliably have blue hearts, so check them out if you're a little low. Arcardes and self sacrifices rooms don't have much value, but curse rooms are pretty good. If you can beat bosses consistently without taking damage, go to boss gauntlet rooms. While their rewards are not as nice as they used to be in WotL, they usually have a good payout.
If you can get the left hand trinket, you should keep it. Red chests are risky, but they can pay out with double blue hearts.
As a decent blue baby player, I can tell you that the first 2 floors are primordial, and you should focus on gathering as much health as possible. Since you will always have a deal with the devil after the second boss fight, you need to be sure you have 3 blue hearts to spare. So I would not advise going for the first curse rooms, unless you can spare a blue heart. Perfecting those floors is also very important.
For the deals with the devil, unless you get uber tems like brimstone and mom's knife, focus more on mobility and survivability items. (void the familiars which are terrible value). Exceptions are made for the pact and the mark, which refund a part of their cost on pickup (making them cost respectively 1 and 2 blue hearts instead of the usual 3).
Edit : If you have a lot of bombs on the depths or necropolis floors, blow up the blue and purple fires. They sometimes drop blue hearts. And the skulls can (rarely) drop black hearts.
One last thing. With blue baby, the polaroid will give you a small period of invincibility everytime you get hit. If you feel your run is not going super well (low life, or not as much damage as expected), take it and go for a chest run.
