Samson didn't unlock
Can someone explain to me why Samson did not unlock when I beat both basement levels w/o taking damage?

because the "xxxx boy" achievements dont let samson unlock. you have to do it a second time

Basement boy = no damage from enemies
Samson = no damage at all (curse rooms, blood bank,etc)
