How to unlock It Lives boss in Rebirth?

Yeah, 10 or 11 times, i can't remember. Many phases of the game are unlocked by defeating certain bosses more than one time.
Try to defeat Isaac and Satan 5 times each.

Basically you just need to keep killing Mom's Heart.
Once It Lives is unlocked, it permanently replaces Mom's Heart (though the game considers the two the same for unlocking things).
Once you get into the groove of going through Sheol or the Cathedral every run, It Lives should be no threat to you. It's just a matter of learning how to dodge the bullets.

Beat Mom's heart 10 times, you should unlock It Lives, Blue baby, and Issac.
