Only pick it once so it registers in your item collection, avoid it at all costs unless you're ???? or Azazel, or if you want to become guppy of course
It's universal that every new player is going to get trolled by picking it up and finding they lost all their hearts. Given the RNG on items in this game, picking up any unknown item is kind of a gamble, dead cat stands out because it really polarizes your run due to taking all your hearts.
But yeah, it's amazing depending on the situation and the character. For advanced players, heart containers become less useful the further into the game you get. They are most useful for early game survival and for the blood donation machines. Trading them away for devil items that will help you avoid damage is way more useful, especially if you can find a way to get lots of soul hearts. Dead cat essentially lets you make every devil deal, which is why it's a great choice in some situations.
