Pyromaniac...
So basically... tons of bombs = I'm invincible. Bob's brain = I'm invincible. Ipecac = I'm invincible. BBF = I'm invincible. Anything that modifies my bombs basically means I'm invincible.

Why bother nerfing Yum Heart when this is in the game?

well ♥♥♥♥, that explains how i was rocket jumping without taking damage, hoped it was a proper distance/skill thing.

Apparently blasts heal you, too, when you have it on. Read that on the wiki, haven't tested it since I only have two hearts. This sounds like a single-item insta-win, provided you can find even one other bomb item to pair with it.

Yeah if you have pyromaniac, damaging yourself with bombs will actually heal yourself. Also any enemies that shoot ipecac willl heal you on contact with the bombs (ipecac) rather than damage you. I think even moms foot stamping on you, and satan too, will also heal you.
Id say the difference though between this and yum heart is that a character starts with yum heart, maggy. So maggy runs were almost always wins. Im guessing thats why yum heart was nerfed, whereas pyromaniac has to actually be found and picked up
Pyromaniac + Kamakaze = Full invincibility.

What was the nerf for the Yum Heart? It still is pretty OP. I once had a run with her that she literally had all the healing items and the heart saturation was so rediculous.

Higher cost. In the first game, if Maggie picked up the Habit and the Wafer, it was absolute invincibility. For every heart she lost, she could gain one back so she could never die. Only the Habit was needed pre-Womb due to the lower damage, and even without the Wafer it still doubled her life.

For reference:
Wafer halves all damage (everything does only half a heart)
Habit puts one charge on your Spacebar item for every hit you take
Yum Heart only needed two charges

kamakaze-space bar to heal, infinite uses per second.

wafer is the op thing if anything, spark plug+ wafer = half heart full item recharge. it isnt hard to figure out whats wrong with it.

Not sure why in the hell this was necro'd, but let me put my input in.

Pyromaniac is a Special Item. Special Items are rarer than normal items, and picking up Special Items DRASTICALLY reduces the chance of seeing other Special Items. There are fifteen Special Items in the game:

Pyromaniac, God Head, Polyphemus, Brimstone, Mom's Knife, Mutant Spider, Lil' Brimstone, D6, D100, Sacred Heart, Epic Fetus, Dr. Fetus, Stopwatch, Soy Milk, and Ludovico Technique.

So if you were ever to take stuff such as Polyphemus or Mom's Knife, Pyromaniac may as well not exist in that run anymore. It's meant to be extremely OP, just balanced by it's rarity.

Wow, Soy Milk? Picking up Soy Milk or Dr. Fetus both craps my run AND lowers my chances of finding a better item?

Pyromaniac is quite a bit more OP than most of the other items on that list. Would you rather have that, or Ludo?

Few thing match the obscenity of Stopwatch. Seriously, pick it up some time. It's absurd.

i wish there wasnt special items such as godhead and stopwatch it means its almost impossible to get those amazing items in one run

It's quite honestly the only way to balance them and still make them hilariously good.
