No, you have to unlock it. If I'm not mistaken, you have to beat Mom's Heart with Maggy.

If you have the option to pick it up, don't if you already have Loki's horns or Mom's Eye. The IPECAC effect goes on all tears!

I had mom's eye which I always thought was useless (but saw no harm in picking up), and picked up IPECAC. Short story, my run ended one room later.

no, just beat moms heart 4 or 5 times, I already have it

but i allread beated the game 5 times (because i looked at the game's stats and i have endings 1 to 5)

Aw, carp, I need to unlock Maggy first...
I'd like to play with IPECAC, too. I lose with it usually, but it's fun. Unless you meet many flies or spiders

Dr. Fetus was always my favourite in the original, IPECAC is a little more unwieldly and less potent, though it does have the advantage of firing in an arc instead of along the floor (which means fetus bombs either getting blocked by obstacles or have a fixed firing distance making it pretty useless against fast charging enemies)..I have to say, I'm looking forward to seeing how tammy's head active+IPECAC works out though. Tammy's head is perhaps my new favourite active now thanks to the one-room charge and insane potency with the right tear modifiers or even just high damage basic tears. Unfortunately, they made Dr Fetus and Epic Fetus (which I was never a fan of anyway) much rarer in rebirth, so I probably won't get to abuse them as much as before. On the other hand there's an explosion immunity/healing item now though I believe...which makes other things far more abusable.
