There's a chance a soul heart or black heart will drop everytime you get hit, I believe.

When you take damage there's a chance of either a soul heart or a black heart (whatever they're called) dropping. On top of that it looks hilarious, I wish I could start every run with it!

Taking damage can allow for the chance of a soul/black heart to drop. Killing enemies allows for half red hearts to drop.
