Slow for anyone else?
I feel like I'm walking through molasses and I don't understand why

Well I'm playing with a controller so it's not a keyboard problem... and there is no input lag at all. It just feels like game is playing at 50% speed even with the audio being fine and dandy

Same problem here. I'm playing with controller on a Linux system.

Fixed it by turning vsync off in nvidia control panel

Tried it with vsync off. In options.ini + control panel but still slowed down for no apparent reason.

Game is slowing down every few seconds. It runs smoothly then suddenly slows and then goes back. Very similiar to the first-birth Isaac. Is there anyone else with such a trouble?

fixed it on my system. I usually have it set to 4k display on my tv which only runs at 30hz and I guess that was forcing the game to be 30fps since I couldn't adjust the resolution within game to 1920x1080 to force 120hz like I do in most of my games. fixed it by going to startup options for game and forcing the resolution and fullscreen "-fullscreen -w <1920> -h <1080>
