I'm only doing my first runs, but mine is
Cursed eye, 20/20, flaming tears, cancer, Some dmg ups, bloated eye, and some item that makes my tears explode randomly + Luck up, and homing tears

Just finished a Cathedral run with Judas. Wafer, Virgo, Black Bean, Prayer Card and Fly Halo. I was invulnerable to standard projectiles, poisoned on touch with reduced damage and chance of shield, no bad pills, and a chance to get the 48h energy pill which pretty much meant an extra heart at the very least.

Not a single tear upgrade. Didn't need any.

Recently I had a run with chocolate milk, hive mind and guppy transformation. Chocolate milk lets you shoot tears as fast as you can press the button, but without reducing your damage stat like soy milk, so you get a crazy amount of full damage flies.

20/20, the inner eye, mutant spider, moms knife, brimstone, soymilk, spoon bender, Libra and blank card jera. all of those were obtained before the chest fyi so.... I got lucky. I the chest I basically got a bunch of dmg up. the next run I got double steam sale on greed mode update, I forgot to mention poly before chest
