Can't acces the chest/dark room while using Isaac

I'have finished the game again and again with many characters but, everytime I use Isaac, the run ends after beating to a pulp Satan or Isaac in the 5th chapter. It's the third run in a row that ends this way and still I dont know why!

Are you picking the correct piece?
Are you picking the Negative and going to Cathedral?
Are you picking the Polaroid and going to Sheol?
If you say yes to either of these, then it won't work if you do

Beating Isaac 5 times unlocks the polaroid item. Beating satan 5 times unlocks the negative item. Both items will appear after defeating Mom. When you pick one, the other will dissapear (unless you use a teleportation glitch). Polaroid is the light picture while negative is dark picture. If you want to go to chest, pick polaroid. If you want to go to dark room, pick negative. THIS IS THE MOST IMPORTANT STEP RIGHT HERE: After beating It Lives! A trapdoor and beam of light will spawn. Trapdoor leads to sheol while beam of light leads to cathedral. If you picked the polaroid and want to go to chest, go into beam of light to cathedral. If you picked negative, go down the trapdoor to sheol. Once you have beaten Isaac and you have picked up the polaroid, Touch the chest and you will be transported to the chest! Once you have beaten satan and you picked up negative, touch the chest and you will go to the dark room! If you beat Isaac and have the negative and touch the chest, run will end right there. If you beat satan and have the polaroid and touch the chest, run will end. MAKE SURE YOU BEAT ISAAC WHILE HOLDING POLAROID AND SATAN WHILE HOLDING NEGATIVE TO GO TO CHEST/DARK ROOM (Respectivley!)

I have unlocked both the item since I have finished the game over and over again, my question is why every run I have tried so far while using Isaac (not judas, maggie, azazel, cain, and the other characters) ends after beating the boss of the fifth chapter.

Really old topic, but I just had this happen to me playing as Isaac. I'm 95% sure I took the Polaroid and went to Cathedral but after beating Isaac, taking the chest ended the game.

This ever happened to anyone? Did I really accidently take the Negative?

Not using any addons. But yeah I tried again and it worked fine so I guess I really did take the Negative. Sometimes these things are weird.
