If Repentance+ isn't installed, then no, nothing changed, 50% chance.

Only if a character already beat Mother before. Which kinda defeats the purpose if you're unlocking stuff.

It spawns after every end boss except for beast now, as long as you have the void unlocked.

Its actually relevant for co-op as your singleplayer mark counts for that portal spawn.
