Glitched Item?

my first start with eden gave me Nail and another weird black and white item like the static of a television screen (its not the Item from Dogma).
it changes your eyes and tear to this static and has only one effect: when i hit an enemy with a static tear a reverb sound appears and the enemy gets switched with a different enemy. like Fly -> mullibomb etc.
it appears to only have this effect (only normal enemies i think).
I cant find this item anywhere and because i started with it i couldnt see its name.
Is it new or bug?

Do you have any mods installed? If yes, maybe it's just that - a mod.

If you are sure, it's not a mod, have you beaten Delirium as Tainted Eden? This will unlock glitched items to rarely appear in secret or I AM ERROR rooms, like the ones you'd get with TMTRAINER and you may just have gotten super (un)lucky to start with one as Eden.
Do you still have the seed? That'd be a great way to re-produce this, if your game isn't too heavily modded and it would generally be interesting, to have a seed like that, because Eden starting with a glitched item from the "Corrupted Data"-unlock seems pretty rare.
Edit: You are able to find the seeds of your last play session, when going to <C:\Users\*UserName*\Documents\My Games\Binding of Isaac Repentance> and open "log.txt" and then search for "RNG Start Seed" (press CTRL+F).

Thats the seed i found also my game is moded but only with cosmetic mods like external item descriptions charge bar etc. its pretty much the mod list from a Youtuber named "Lazy Mattmann".
I will send a link to a video i made (when its finishes to render in youtub) with obs for this (i know its ♥♥♥♥♥♥ quality, but i dindt plan doing one,its for evidence and maybe help if somebody knows this items)
also update: the Tear does not only switch the enemy hit with another enemy it switches it with Everything like pills, dead shop owner etc.

That's the item Segmentation Fault[tboiepiphany.wiki.gg] from the Epiphany mod.

oh my bad than i didnt thought the cosmetic mod added something then

The costume and effects sounds like you got "Segmentation Fault" from the Epiphany mod, which occasionally makes Isaac's tears reroll whatever they hit as if you used the pre-nerf D10.
