Help with Book of Virtues

Can someone explain to me how Bethany's active item works?

Sometimes i can spawn only 6 orbitals and other times i can spawn much more, what affects that?

It all depends on what active the book is combined with. Different actives create different wisps and there are 3 separate orbital rings with different caps per ring - some can only spawn on a specific ring. Some also only produce temporary wisps or wisps that don't act as orbitals altogether. The wiki is probably the best place to check if you want to see synergies with a specific combination.

i had a run with bathany where i got from an angel deal that item which makes you over time invincible aslong you hold the spacebar down. i forgot how it was called but if you ever SEE that thing TAKE IT! cause when the invincible runs out but you keep holding down the spacebar spawns it infinite wisps, it was amazing xD the whisps have the effect to protect you from any damage etc.

tho even after daemo explained it understand i the item still not either. i had a run with book of virtues+vortex. the whisps didnt seem to do their effects over whatever vortex nommed up

i just think some stuff works, some stuff dosnt ._.

Considering Breath of Life is arguably the biggest dud in the Angel Room pool in any other situation...

There's far more OP book of virtues combinations anyway, especially combined with battery items like 4.5V, 9V and car battery to minimise or even straight up remove the need for soul charges to keep item effects online permanently.
Unicorn stump and book of shadows come to mind as literal god mode options once you've got the orbitals up, both made even more ridiculously potent with the panic button trinket. My little unicorn works just as well too, though may cost a few soul charges if used too much outside of bosses.
