Got Vanishing Twin and Great Gideon in the same run
Cool bug, Edmund. Tell you what, why don't you make the key and knife pieces get affected by Sacrificial Altar? While we're at it, why not make Dad's Note get affected by Chaos? Afterall, getting softlocked due to TMTRAINER isn't enough. Hey, actually make it so at the end of it lives, only one path spawns instead, 50/50 either Sheol or Cathedral. Oh, and if you get to mom's heart within 20 minutes, have the boss rush door spawn instead, and make Hush's door not spawn for the rest of that run. If a void portal spawns, please teleport the player straight to the void, don't even ask him, who doesn't like Delirium with base tears and damage?
And I've got plenty more such wonderful ideas, I'll write them all down once my hand starts bleeding from all the wall punching I'm gonna do.

it's bugged as the second gideon clone counts as a enemy and the waves only spawn when there are no other enemies in the room. So because there's a second gideon in the room it counts the wave as not being complete and will never complete unless you have a chaos card and use it as the gideon chaos card thing still exists apparently.

Ignoring the insane anger supplied from the post, I'm going to say this now. Bugs are going to crop up, and I don't think it's fair to say that Vanishing Twin + Great Gideon was a chronic gamebreaking bug. It was just an oversight that didn't come up in testing.
I'll report the bug to the devs. It's an annoying softlock, but not alot more.

I get that not everything can be tested before release, and this isn't TERRIBLE (since it's sorta unlikely), but yeah, some of the bugs just seem like passive things that come to mind when picking up an item or fighting a boss.

I used an Ehwaz in Corpse II, it worked, I thought I discovered a secret to loop the floor, and then the game froze after a loading screen. It has been patched since, but did nobody think to test if a new "final floor" works as intended with trapdoor making items?
