How do I access the new floors?

I have the new expansion, and I know about Dad's Note, but the other floors are a mystery to me. Any help please?

Thats it, the whole alternate path becomes available. (Provided you have the resources to open the doors to them, anyway)

New doors will appear in each boss room to the next available alt floor. The requirements for opening those doors will be very clear.

Depends on what you're talking about.
The alternate route which is Downpour -> Mines -> Mausoleum is acessed by using resources on special doors.
On Basement/Cellar/Burning Basement I or II, there'll be a Grey door which requires a key.
That door leads to Downpour (Or Dross if you have unlocked it)
On Downpour/Dross II, there'll be a Mirror. You have to get inside it by touching a white flame and turning into a "The lost" version of your character.
Then you go to the treasure room. (You can leave the mirrored floor on the mirror)

After that, to go to the Mines (Or Ashpit if unlocked) you have to bomb a door on Downpour/Dross II.) 2 bombs are required or 1 only if you have Mr. Mega.

On Mines/Ashpit 2, you need to press yellow buttons that appear scattered on the floor to build a minecart track that leads to the secret area where you get the second part of the item (This area only appears if you got the item from Downpour/Dross)

After that, finish the floor and spend 2 hearts (full ones, can be normal, black or soul)
To enter the door to Mausoleum (Or Gehenna if you have it unlocked)
After that, just proceed normally, after the boss figt on Mausoleum/Gehenna 2, there'll be a door which requires the complete item from Downpour and Mines.
Heading there will make you fight against a boss. After you defeat that boss, you gain access to the new floor "The Corpse" then just go normally from there, Corpse 2 is the place where you fight the main boss for that alternate route.

If you're talking about the ascent, just get to Depths/Necropolis/Dank Depths 2, beat Mom, go back to the starting room and use The Polaroid or Negative on the Strange Door. This strange door only appears after you complete the alternate route at least once.
(Also, there'll always be a skull with an X marked on it on the floor and it always gives the 0 - The Fool Card)

Must I beat Hush 3 times for Dad's Note to even appear? I tried to get it, but I didn't find a door that has it at all.

You beat Hush 3 times to unlock the alt path.

You beat the alt path once to unlock Jacob & Esau as well as the strange door in Depths 2.

Do previously said steps to open the strange door.
