Guppy paw, guppy head or shears?

I am on the second floor, got the shears on the first item room, got a 3 red chest devil deal, two were the above guppy items, the other one was spirit hearts...I'm probably not going to take guppy paw, is head or shears better?(using isaac without d6, have 4 hearts and 2 spirit hearts)

Make sure you pick up and put down the guppy items. If your damage is "ok" to "good" take the head. If it's "bad" AKA Eve Level, take the shears.

Just to clarify what the guy above said, even if you're not going to use/take a guppy item with you, make sure to always pick it up, unless it's the dead cat when you have no health at all aside from red hearts. The reason for that is the guppy transformation. It's a pretty common subject, so you probably already knew this, but anyways, there you go.
