Help me understand re-roll
I just don't understand the value of this item (D6, D20, etc.). That said, it seems to be a favorite for many so I must not understand how to use it.

Once I have a build I'm comfortable with mid-game - I just want to take it to the end.

Sure, I'd like to switch some enemies on the occasional nightmare floor I hit but not that often to make the item valuable.

This goes for ''red rooms" as well. I almost never go to the middle of the room to re-roll because I'm familiar with the build that i used to get me there and instead hit the chests or other items on the sides of the room.

What am I missing? How can I use this item to its fullest?

Uhm are you using the Dice items properly, like using D6 to reroll pedistals, and D20 to reroll consumables, etc.
Also, it's because we can reroll a ♥♥♥♥ item like Flush into Proptosis or something.

D6 is one of the best items in the game simply because it lets you re-roll pedistal items that you would normaly just skip. (Pedistal items include treasure rooms, curse rooms, the shop, the devil deals<switching a 1 red heart devil deal into something op is always fun>, god room, and the rare pedistal sitting in the middle of a room for no apparent reason)

D20 can be super op when used in a arcade room. It changes any dropped item(non pedistal) into any other dropped item. Which means you can get a ton of coin/heart/bomb/key drops then reroll the entire room for free chests (and possible pedistal items from said chests)

The others i dont use too much but they can be situationally good.

The different dice work differently. If you use them correctly they can all work to your advantage, potentially to the tune of several extra good items; if you use them incorrectly they range from useless to devastating.

The D6 is easy to use--it rerolls any items in the current room that you haven't picked up yet, so use it whenever you see an item that you don't want (because it's underpowered or doesnt work with your build or you just don't like it).

The D10 is mostly pointless--it just replaces the enemies in a room with other enemies. Good if there's particular enemies you don't want to fight, otherwise junk.

D20 rerolls pickups in the room (ie hearts, coins, bombs, etc). It also rerolls trinkets. Generally it's best to use this after opening a chest after grabbing everything but the pennies, to replace them and the empty chest with more bombs/keys. Or in an arcade after spawning a ton of junk on the floor.

The D4 is the one that rerolls your whole inventory. You probably don't want to roll it near the end if you're happy with your gear, but early in the run it can be helpful if you know that the items you have aren't very good--just keep rerolling until you get a couple good ones (or you think that you'll lose track of which items you really have if you continue). It actually tends towards a net profit as long as you don't exhaust the item pool (by rolling it many times with a lot of items), because many items give you stuff when you first pick them up (bombs, keys, coins, soul/black/eternal hearts). None of those things are taken away when you reroll, but they are given again for the new items. (I know that the same is true for health items if you're the blue baby, don't know about other characters).

The D100 does the D4, D6 and D20 effects all at once. Which really just makes it a gimmicky version of the D4 since that's the biggest effect.

Dice rooms depend on what number is on the die (the map icon doesn't tell you anything, look at the floor of the room). 1 pip is like the D4, so beware. 2 pips is like the D20 so it's safe. 3 pips is the D20 for the whole floor--always enter the shop first and check back after rolling, because it has a chance to replace 3 and 5 coin purchases with items. 4 pips does the D6 effect to the whole floor, so use it only if you saw an item you don't want. D5 restarts the whole floor; if you use it after the boss and the item room you can get an extra level's worth of items. 6 pips does the same effects as 1, 3 and 4, which again really just makes it a gimmicky D4.

Or the short version on the dice rooms: Avoid 1 and 6, always do 5 after getting all items on a floor, do 3 for a chance at discount items in the shop. 2 doesn't matter, 4 only matters if you see an item lying around you don't want.

At the moment i am re-rolling using isaacs D6, i opened a shop on the 1st floor and re-rolled the passive items in that shop to the Habit and sharp plug... bout time

Now i need a few more things to begin the SMB&BG achievements

Alright, i just re-rolled a tammys head for a monster manual :D its a great mechanic in the game but sometimes a two edged sword...

flush actually isnt a ♥♥♥♥ item, it comes in handy in a huge room but if you have d6 in the hand you should reroll it
