Book of Revelations doesn't actually guarantee a horseman?

So the Book of Revelations item, in both the original and the remake, is supposed to have a second use in that it guarantees that you will face all four Horsemen and thus will get a level 4 Meat Boy or Bandage Girl (and the acheivement). It's marred by the fact that the game allows the Headless Horseman or Conquest to replace a rider and if that happens in Depths or Womb you won't get another chance at the meat cube.

That's how it's supposed to work.

So I was more than a little annoyed when I got to Womb 1, holding the Book of Revelations, and the boss I encountered was... Mr Fred. Not one of the horsemen, not one of the bosses that's supposed to be allowed to replace the horsemen, just Mr Fred. There goes my acheivement and my guaranteed powerful item.

Has this happened to anyone else? (I think I *might* have unlocked Conquest a few levels earlier on that very same run, which could be related to the bug)

Did you USE it before fighting the boss or...?

Oh. I didn't realize that you had to actually USE the book before reaching the boss. I already had a full health bar, so I left the book sitting at fully charged so I could use it as soon as I took damage. Now I feel sad.

Yeah you do.
It's even more disappointing when you get Book of Revs and you have Meat Boy/Bandage girl ALREADY, and you get the Meat Cube when you needed the Bandage ball.
Luckily, I got both achievements eventually.

That can happen? I'm yet to get a meat cube when I had bandages or visa versa, so I just assumed it was guaranteed to give the one you already have (ie only one of them appears in the item pool per game).

Yeah only one shows up, so if you fight famine and end up getting Meat Cube instead of Bandage Ball you're ♥♥♥♥♥♥.

The problem isn't that you didn't use it (well, you needed to anyways, but it didn't matter either way in this case), but that Mr. Fred always inhabits a double-sized boss room which is set when the level spawns. The Harbingers cannot spawn in these double rooms.

... At some point the spcial cases start outweighing the "normal" case and you have to wonder how he expected anyone to get those acheivements.

In 120+ hours of playing Rebirth, I have never seen this happen. I'm almost positive that once you get one, either cube of meat or ball of bandages, all of the horsemen will give the same.

Also, if you do have book of rev and get three cubes followed by Conquest, all is not lost. If you can pick up a Monster Manual in the chest, you can try to use it as many times as you can to get the fourth cube or ball. The achievement unlocks despite the fact that the fourth level is only around for one room. This is how I ended up getting both Meat Boy and Bandage Girl.

Edit: rereading, it looks like the two of you are talking about different things. Scoots is talking about after you unlocked Meat Boy and are going for Bandage Girl, getting a cube of meat ruins your chances of getting Bandage Girl that run. Brickey is taking about getting both a cube of meat and a ball of bandages at the same time, which through horsemen, can not happen.
