After the 11th time, you get a harder version of Mom's heart, as well as guaranteed access to both Sheol and Cathedral, and Andel/Devil rooms will be normal as opposed to Cathedral/Sheol access, respectively.

After defeating Mom's heart/her buffed version, you unlock the third alternative floors if you're playing on at least Afterbirth+, just remembered because I had them initilally unlocked in Afterbirth

11 times for Burning Basement (also unlocks harder Mom's heart)
16 for Flooded Caves
21 for Dank Depths
30 for Scarred Womb

I believe it's 11 or 13 times to unlock everything from killing Mom's heart. More paths beyond the heart will become available, leading to more bosses, challenges to overcome, and more items to unlock. Hint: each character has their own unlocks for their completion marks (that piece of paper towards the upper left of the screen that gets marks for each thing you complete as that character), so even if you did something like kill one of the bosses beyond Mom's Heart, doing it as a different character will unlock something else

Hm, so doing different chatracters for moms heart unlocks more challenges?

The challenges are unlocked when you unlock said character and have Mom's Heart alt unlocked, the reason having the character unlocked is because you play as a specific character (ie Suicide King has you play as Lazarus), or you have to unlock the Mom's Heart drop (Demo man requires Dr. Fetus, 9th drop, Solar System requires Transcendence, 3rd drop)

Sorry for double post, but a bit more elaboration and a tip from the angel/devil room advice before unlocking the alternative

If you do manage to get the angel/devil room after the Heart fight, you will be taken to Cathedral/Sheol, respectively. If you defeat the boss there, you still get progress towards Polaroid/Negative (each one requires you to beat Cathedral/Sheol 5 times), which gives a buff when you take damage with <1/2 red hearts (temporary invincibility for Polaroid, and Necromonion/Black Heart effect for Negative) and is required for access to Chest/Dark room. If you do manage to beat Sheol, you can also unlock Judas early, who starts with the highest base damage and Book of Belial, which further increases damage, but only has one heart container at the start.

Just keep going through runs, after winning with different characters/ items you'll unlock more endings and characters. You can look at the binding of isaac wiki to see how many different endings there are and how to get them. When i first started, the best way to progress was to take devil deals.
