Grossest thing in isaac

For me, Ive always found mega fatties throw up attack super gross as it looks more like he's ♥♥♥♥♥♥♥♥ from his mouth. Although that may be what hes actully doing.

What do you guys think is the grossest thing?

Edit: You guys can include disturbing or morbid instead of gross if you want.

I'm not really grossed out by anything in Isaac, to be honest.
But if I had to choose, it'd be Harlequin baby, mainly because I know what one looks like in real life.

That is a really subjective question. For me just because I know and have seen the real world counterpart, Fistula is the most disgusting thing I can think of.

Nothing in the game really seems that gross. But yeah, Fistula in real life is probably much worse than in game. Maybe if you think of the blue womb as blue waffle it could be gross.
