Trading your life for items. As in not just losing health but losing the entire container of the heart.

After beating a floor boss, you have the chance for a devil deal which will appear next to the boss room, the devil deal may contain powerful items that require you pay heart containers for them, other times they will contain red chests or black hearts. The chance of a devil deal is based on certain feats and actions you perform, i.e taking no red heart damage, having the pentagram etc. If you refuse devil deals, you may be in the running to receive angel rooms in place of the devil rooms. the difference with angel rooms is that the item is angel themed as opposed to devil and is also free to take.

There are also other methods of entering these rooms, but ill let you work that out for yourself.

Yeah potentially it has the best items in the game that will usually make it much easier/more fun

If luck smiles on you, you won't need to search - the door will appear with a distinct sound in the boss room. However, it is chance based(with a range of modifers depending on floor you're on, red damage taken during floor and boss, various items etcetc), so won't always show.
If it doesn't appear , there's a few ways you can teleport there, or if you find the one item that guarantees the door to show up, the door will be there when you return to the boss room.

BoI is a cruel game because of the devil deal: if you are not good enough, you will not spawn them. If you manage to open them, but can't endure the loss of hearts, you will die.

But if you can plai with it... you will become powerful. And maybe beat the game ^^

Sometimes there's also a boss in Devil Room, called Krampus - he can be quite tough for a novice player, so be prepared when you walk in.

Note that angel room items are of a generally less useful than devil items, mostly giving defensive abilities like flight and blocking shots. The few very good items like sacred heart and godhead are pretty rare, but if you're creative with the right item combinations you may be able to get them by chain rerolling items or spawnign a lot of pedastals with suicide king
