Anyone knows what this knife is?

Well, the question I mentioned basically. I was doing the high brow challenge, got a couple of range ups, and now I have this odd yellow knife following me.
It follows right behind me, and does contact damage.

It is not an orbital, and does not move like a familiar does. It remains right behind isaac, aiming in the opposite direction of his movement.
