Also, there's not really any difference to the screen and whatnot (options and such), because the DLC is just added content.
I know what Afterbirth is supposed to be, but for some reason I can't access it.
I know it may be stupid but it happens. Is the little box next to "Installed" checked?
Maybe verify your game cache's integrity. As Flavor said there is definitely a clear indication on game start up to let you know it's Afterbirth.
EDIT: okay nevermind, it sounded scarier than it was (for anyone with the problem, right click on the game in the steam library, click properties, in one of the tabs there is a button titled "verify cache integrity").
So indeed 1 file wasn't validated, and after this, I had Afterbirth in the title! I believe I was playing the game when the update finished downloading and installing, so I suspected that maybe there was some syncing up shenanigans but had no idea how to fix it. Thank you very much for your help!
Right click->Settings/Options (idk which one)->Local Data->Verify
