Never take Brother Bobby unless you're trying to just get rid of all your red hearts and there's nothing else.
Never take the Shovel (We need to go deeper)
I've never taken the Razor in a devil Deal, but I can see it being useful in some situations
Always take Ceremonial Robes and Abaddon
Almost always take the Pact and the Mark
Almost always take Brimstone.
All the others are pretty situational.
I mean, if there's nothing else on satan-sale, where else to spend all these useless heart containers?
"We need to go deeper" *could* be useful, if using it would fully count as a "completed level" in the game, which it sadly does not.
So stay away from it, unless you want to speedrun.
Obvious "must take" choices would be any guppy-item and damage-ups. The latter usually gives you some dark/soul hearts anyways.

Everything else in the pool is good - amazing.
I usually do, too, though I will admit it can screw up things in a very, very minor way. If you are having one of those unlikely really weak damage runs and happen upon a lil brimstone, I would much rather have him next to me for aiming.
Having a long string of mostly useless companions (bro/sis/chad/etc) and forcing him to the end can make it rather annoying to aim, especially near the edges of the screen as you try to pile them up.
Of course that's a pretty rare thing, so yeah, it usually doesn't matter =P
That said you should always take a deal if your deals aren't locked in yet.

Everytime you get hit you have a chance to drop a spirit heart and enemies that you kill can drop half red hearts.
I would also make a case for the pact and athame as they are both pretty good in my opinion.
Agreed on taking lousy familiars being annoying when it screws up your chain of babies. As Lilith I'd rather have two Incubi right next to me rather than the second being at the end of a long chain.

Well to be honest. You can make a case for any item in situations. Lord of the pit as Maggy could be one of those. I'd pass on it.
As for the pact, yeah, it adds to the Leviathan transformation. tbh though, the Pact is pretty good imo.
