The Voice?
I apologize if this isn't new and I'm late to the party, and will delete thread promptly, but in case this is new..

I got an interesting thought bubble between floors playing as Maggie as I was loading into The Blue Womb. It depicted Mother sitting in a chair hearing the Voice of God from above, then it panned up into the Attic, where the voice was coming from The Lost.

So if I'm getting this right, Isaac's dead womb sibling/s ghost is the one telling Mother to do these horrible things to Isaac?

Well, strictly he hasn't fallen down a well and broken his spine for all we know, so those are probably more Isaac's fears than actual things that happened.

Not exactly what I meant. I think it's obvious that The Basement and such isn't actually real.
If I've pieced it together right, here's a timeline:

Isaac's father leaves him and his Mother, possibly due to the death of their 2nd pregnancy (ending 13).The ghost of Isaac's dead womb mate (The Lost) haunts the attic, and pretends to be God to convince Isaac's Mother that Isaac is a demon child, and to torture him, perhaps in revenge/envy. During this time, Isaac reads the bible, and comes to think that perhaps he really is a demon spawn. He then sees his Mother go for the knife to kill him in the name of God, and he hides in The Chest. In here is where most of the not real game action takes place, where Isaac goes through a gauntlet of his own fears and insecurities. During this time, he's struggling with the idea that he is a demon child, before he gives into it, and accepts it. He stays in the chest, and eventually suffocates to death (Blue Baby). Mother opens the chest and finds his bones there (ending 17), puts up missing posters to cover his death, and leaves (ending 18). Meanwhile, Isaac emerges from the chest in some different plane, possibly some kind of purgatory, before he is taken by a dark, fallen angel esque figure, implying he was a demon child and was taken to Hell.

I... think the game's about escaping reality or something, some dude made a forum post about it a long time ago. Mcmillen approved so I guess that's the closest to canon you can get. It's all very abstract, I think.

But sure, whatever floats your boat.
