Currently Afterbirth has a bug where if you use an imported save you can't view Ending 17, because he gives you a path to the other 2 main bosses.
That area is called Blue womb, at least in the achievements. We know that ??? Is suposed to be the corpse of Isaac. Also, we discovered in the Chest ending that Isaac had a sister. This gives me 2 ideas.
1. Hush is a mental representation of Isaac's dead sister.
...or...
2. "Isaac" is actually his own sister and Hush is just another mental reflection of him/her dead self. Edmund could have lied to us the whole time.
Hush seems to be grown into Mom's womb, like the lump of cells is soon after a fertilization.
I guess Hush might resemble a unborn child of an early stage which "suffocated" after he stopped receiving oxygenated blood because his mother died (after her heart stopped beating).
I think Isaac died a long time ago by suicide for some reason and the mother is growing increasingly deranged because of it, being forced to relive over and over the hardships and suffering she has made her child go through. The Hush is one of the final representations of that; she has to face the fact that her child died, that he was doomed to die from the moment he was conceived because she was such a terrible mother, hence the decomposed child in a suffocated womb. She can't stop reliving this because Isaac is not yet at rest. His spirit is still there, whispering to her.
But, that's just a theory. That is probably my favourite part about this game, I love that there are so many possibilities to the story, and that it can be interpreted in so many ways!
