Transformation doesn't work

Hey guys,
maybe someone can help me.
I just did a greed mode run with Judas.
In the first Item room I got the Pentagram item.
On the other floors I did deals with the devil to get Abaddon and Lord of the Pit.
According to the binding of isaac gamepedia and my knowledge of all the other transformations I should have turned into Leviathan.
But I didn't. Now I wanna know why. Is it because I got Pentagram in an item room?

I might be wrong, but I have only gotten leviathan by making three devil deals, I have only gotten it twice though, so it might have be a coincidence

from what i understand lord of the pit doesn't help get the leviathan transformation.

yeah, I think the problem is that I didn't make three devil deals and got one of those items in an item room
the Seraphim transformation worked perfectly fine after collecting three "holy" items
