How to get the ball of bandage?

I've been trying to get it for a week to get an achievement, but I can't find a SINGLE ONE. So tired of this...

The harbringers usually give you one.

welcome to the club, the usual method is reroll with the d6 until you get the book of revelations and pray you win the crapshoot, yes it's still very much RNG based and a total crapshoot even then.

It's fairly easy with Lillith, if you find just one Ball of Bandages and a 48H Energy pill OR doing a Boss Rush OR doing Greedmode.
There are several other ways to achieve this with Lillith or Box of Friends (if unlocked).

I had Lillith, a Car Battery and a ?-Card and unlocked the Super Bandge by accident.

If you don't have Afterbirth or dislike playing Lillith, then yeah, it's still i pain in the buttocks and Book of Revalations on floor 1 is pretty much your only "chance".

That's exaclty what I'm trying to do but I get Cubes of Meat every damn time...

There's nothing you can do to make it spawn Bandages instead of Meat. Just keep resetting the first basement with Lilith until you fight Famine and cross your fingers for the Bandages. Good luck!

it was actually today (well at least for me) in fact, I think I ended up with two fully levelled bandage girls in BR, lol

You mean the "SMB Super Fan"?
You get that from the "It's in the Cards" challenge. (sadly, there's no Meatboy challenge in Rebirth )

The achievement for building your very own Meatboy is different and doesn't unlock a new item as far as i know.
