D6 is a fantastic item, yes.
Blank Card is very situational, it's rarely godtier, since the chances of a Hierophant showing up are rather rare.
The candles are also situational, but good for low damage runs.
D100 is once again, situational, as your run might be too good to reroll.
Guppy's Head is really only good for low damage runs, or characters that start with no spacebar item/A bad spacebar item.

You don't need a Heirophant to make Blank Card god tier. Blank Card is the main game breaker. You can get 1 of many cards or runes to make it good.

Guppies head is good for any run. Spawning flies every room that do 3x your damage is never a bad thing, not only that but it adds to becoming guppy.

These items are definitely more useful than a 15 room charge.

Actually it can't do that much damage as it's damage is not constant and Hush and Ultra greed scale with damage

I would say that things like the D6 and blank card + chaos card are better. D6 has potential to make you way more powerful throughout the run and blank card chaos card trumps literally everything. Mega blast is definitely good, but the charge time keeps me from calling it the best.
