Its an item That, I think that when you get hit you start looking like you got the star from super mario world, your invincible and can damage enemies by just touching them, and while in this state you can't shoot.

That is a spacebar item called Unicorn Stump, It is a 1 room charge. You must activate it yourself though.

It's a trinket called Gamepad, every time you get hit you have a chance to go into a Pacman animation which grants you invincibility and damage on touch.

Unicorn Stump has a 1 room charge and you go invincible, but you can't do damage (unless you have an orbital)

Old Cartridge can randomly turn you into Pac Man

Gamepad lets you make yourself turn into Pac Man

My Little Unicorn lets you make yourself invincible and you hurt everything you touch

Taurus makes you keep speeding up until you get the My Little Unicorn effect (except it looks a little different).

This must be it. I looked it up on google and I remember getting that Item. I literally decimated Mom's heart with it. The only thing more powerful than this must be Brimstone+ tammy's head.
