I think a lot of these "room-effect" ones are global.

With one ??? character others got flies from destroying poops and others got coins from destroying rocks when one of us had a shiny rock. I assume it's the same for most of these items.

yea that happened to me too
idk i tested it more and it seems to be global but that trinket where it drops keys when picking up coins seems to work only with the person that has it

Its in the "global" item list since it affects the tile itself.
