Room layouts in general are a lot harder too overall, especially when doing the alt-path.
Should've stuck to normal-mode, where none of that's an issue, until you've gotten used to all the Repentance mechanics and changes, like Angel Rooms being actually good (and often the more consistent option).
I don't think you know what the word fair means. There's plenty of rooms that can have unavoidable damage in Isaac, however all the games you listed are consistent. There's no RNG in them so fairness doesn't even come into play because there will never be a scenario of unavoidable death in any of them.
I like Isaac but I'm not gonna lie about it.
