Real Platinum God achievement

What is required to get it?

Everything unlock able in the game. All secrets, all items, all hard completion marks as EVERY single character and a whole lot of rage

191 is beat hush as keeper I think

Yes, even the keeper. Check your completion marks. If you are missing one then that is the secret you are missing

To get Real Platinum God you have to unlock every ending and unlock and pick up all the Rebirth items including The Lost's pickups. To get 1001% you have to do all of the above + unlock and pick up all the items in Afterbirth, including Keeper's unlocks, get all the secrets, and complete every character including the Keeper's post-it notes on Hard Mode.

Also #191 is beat Hush with Keeper, #217 is beat Mega Satan with Keeper, and #84 is Real Platinum God.

Just to make sure I understand:

Real Platinium God is 100% completion without the Afterbirtch DLC, and is acheivable in the vanille game (and represents 100% completion of the Vanilla achievements and unlocks)?

You understood it almost perfectly. Unlocking every item is not enough, you have to pick up every one of them at least once too.

I'm wondering if you have to do all marks from ab and ab+ and mega satan with rebirth characters or if you can miss those out and just do mom's heart, isaac, ???, satan and the lamb

Rebirth marks should be enough for the Real Platinum God... except for the Lost.
See, back on Rebirth, the Lost was the only character who had a "do all marks on hard" unlock. This requirement never changed with AB and AB+, it's still "get all marks on hard", including new ones added by expansions.

Anyway, that's hardly relevant. If you have AB+ your actual end goal achievement is 1.000.000%. Any other "bunch of stuff done" achievements will be unlocked on the way.
