I personally prefer Cancer over Curved Horn, but they are (for me) the 2 single best trinkets in the game.

if you notice in his videos his steam name is Cash Cab or something like that, it shows on the leaderboards thing, anyway, don't make me regret stating the obvious there, but as to ontopic, cancer by miles in my opinion.

well the thing about it is cancer is easily one of the best fire rate ups in the game, curved horn is additional damage, meh, i can get that anywhere and frequently do.

in fact the only reason i could see taking curved horn over cancer EVER and i mean literally the only situation is if you're already soft-capping damage and need to push it just a point or two further, assuming curved horn can even break into hard-capping.

Also yeah, curved horn is pretty sub par compared to cancer, I just had a run with cancer where I had near soymilk tears even without mom's box.

When you have high damage (Poly, quadra shot etc.) then good is cancer, when you have low damage but tears as hell better is curved horn.

go for cancer, it helps you more the more tears up you manage to find

I think cancer is objectivly better unless your damage is total garbage

Curved Horn is better if you have a really low tear stat, because the higher your tear stat the more of a boost it gives you, so the tinier your tear stat, the worse your boost is. Curved Horn is always a +2 damage up, (Unless you have Mom's Box) regardless of your damage.

I don't know how much Cancer could help you in that situation, but Polyphemus isn't THAT powerful. Most things in the game will survive 1 hit from a Polyphemus tear, so I would go for the 2 damage up via Curved Horn.

For me it depends on the situation, though it's rare enough to have a situation, where you need to decide between both.

When your tears are capped and/or have effects, like Holy Shots ---> Cancer.
When DMG is softcapped and/or multiplied or you can charge your shots ---> Curved Horn

Or hope for Mom's Purse if you manage to find both.

But which does flat out increase your damage per second the most on it's own?
Curved Horn, if all stats are at 1 without any items.
