How do i unlock pica run?

i have most of the achivements but i can't get the picarun challenge, any help?

I believe you need the marbles item in order to unlock pica run, which is unlocked by eating 5 gulp pills in one run. Hope that helps.

Its legitimately one of the hardest achievements in the game so you'll be there for a while.

The only way to force a Gulp pill to spawn in the pill pool is to hold R with Lazarus until you start with one. Then pray you get Little Baggy and/or Placebo.

Pretty horrible to get but you will achieve it eventually.

I made a "pica run" mod in case you want to try this challenge

i was always doing it as lazarus (so that his starting blue pill is gulp, that's already +1 use and you are guaranteed to have gulp pill in your run) in greed mode. stack up money, reroll until pill slot in the shop and pray for the blue ones. of course over the course of the run, parallel to getting items to survive. also placebo will help a ton
