gaining flight and then losing flight

i have no idea how, but sometimes when i get flight items, i lose the flying effect that they give randomly. is this a bug? im not playing with any mods. also sometimes when i have flight items the ghost tail changes to wings and idk why. pls help

Any self-reroll effect? D4, Dice rooms 1 or 6, MissingNo...
Do you use Clicker? It removes an item on use.

did not have or used clicker, no reroll items. i had Empty Vessel, but im pretty sure it happened to me with more flight items.

Empty Vessel gives flight only if you have no red hearts, so that might be it
