I am confused how I am supposed to get my hands on this item?

That is one of the troll room layouts, with an item that is not easily accessible.
In this case you'll need items like:
- Stitches - teleport to specific tile
- How To Jump - make Isaac jump, works with this layout
- Mr. ME - summon a ghost that brings item pedestal to you
- Moving Box - pack up to 10 pedestals or consumables, and unpack in a different room
- Diplopia - duplicate an item, can be used once
- Crooked Penny - same as Diplopia, can be used many times, but there's 50% chance everything in the room will vanish and be replaced with single coin
- Pluto or One makes you small pills to lower your hitbox and pass through the obstacles. Use with caution as you can go OOB and end up in I AM ERROR room
