The intro and the first ending (when you beat the game fir the first time) are basically a summary of the biblical "binding of Isaac" (very loosely adapted).
And that's pretty much it.

The premise of the game is highly inspired by the actual story of Abraham and Isaac but the whole lore isn't necessarily based around it if that's the question. This game is basically about the inner struggle going on in Isaac's mind as he is about to meet his death, to put it simply.
